Please arrive 10 minutes before your scheduled appointment, as this will allow adequate time to get checked in and complete paperwork if needed. If you are unable to keep your appointment, please call 24 hours in advance to reschedule so that we may provide that time for another patient.
If you are more than 15 minutes late for a scheduled appointment, you may be asked to reschedule. If you know you are going to be late, please call the office to let us know. This will enable us to see patients as timely as possible. Late patients will be seen at the discretion of the provider.
If a patient fails to show for 3 appointments, the family will be notified by mail. Repeated no show appointments may result in termination from the clinic.
Please bring your insurance card to the appointment. We accept most major insurances, including, but not limited to, Aetna, Blue Cross Blue Shield, Champ VA, Cigna, Community Care-HMO, Coventry, First Health, GEHA, Global Health, Healthchoice, Mutual Assurance, Preferred Community Choice, Soonercare, Tricare-standard and United Health Care.
